Sudomotor testing assesses small fiber autonomic nerve function by measuring sweat gland activity using non-invasive, painless methods. Because sweat glands are innervated by unmyelinated C-fibers, sudomotor testing provides objective insight into peripheral autonomic function in as little as three minutes. Small fiber dysfunction is often one of the earliest manifestations of peripheral neuropathy and may be present even when symptoms are minimal or absent, making it difficult to detect through routine clinical examination alone. Sudomotor testing supports the evaluation of autonomic and peripheral nerve function and can be a valuable tool in the assessment of patients at risk for neuropathy, including those with diabetes.
